This is a little understood part of the benefit of states not expanding Medicaid.  This may seem counter intuitive to most the marketplace, however, for providers this may be a great opportunity.  It also may provide a better product for those who need medical benefits and may have been limited in treatment options under the old Medicaid system.

Scott Walker may have really done a great favor to the citizens of WI on multiple levels by not expanding the budget bloated Medicaid program.

Why So Much Recent Fuss About Health Care Costs?

Todd Hixon discusses a unrealized aspect of the Affordable Care Act, the shifting of spending at the provider level.  There is a unscientific rule in life which applies to economics called the "The Law of Unintended Consequences".  Maybe you are familiar with this life principal when you thought you of a time where you thought you were doing the right thing in a contrary situation to achieve the best outcome only to find out days, weeks, months, or even years later that what you did actually ended up hurting the situation.

I believe we have an example of that situation unfolding with regards to healthcare costs and the provider experience.  If health plans sold via the exchanges have high/ rich benefits what reason would I have to shop for the lowest cost?  Similarly, if I am a provider that is now being paid at higher than Medicaid levels (in states where Medicaid was not expanded) I am going to be getting paid 70%-100% more for patient treatment than I was previously.

How are we going to afford this type of change without a method to have market forces drive care/ treatment to those with the best cost and efficiency outcomes?

Oregon has published their proposed rates for small group and individual health rates in the new exchange.  It is interesting to see the differences in the individual and group rates between the carriers.  The most shocking aspect is the difference that some individual rates are that are lower than the group rates.  I would not have expected that type of a difference in premium.  Especially when considering the individual premiums may also be even lower due to receiving a premium subsidy!

The interesting piece of the attached article is that it provides a comparison of the differing cost of healthcare across our country.

I live in WI and we have, arguably, very good healthcare.  However, our cost would tend to support that assessment.

From a different perspective, Vermont also has a similar cost per resident as Wisconsin.  I am noting this because VT has announced their Exchange rates and Qualified plans (Platinum, Gold, Silver, and Bronze).  I wonder if in WI we can use their announced Exchange rates as a benchmark for our own potential numbers?

This subject is going to receive more publicity as we move forward into Obamacare.

The reason is that there are some excellent financial rewards within Obamacare for introducing a wellness program to your company.  The catch?  You have to have a group health plan.

Wellness is good, helping people change behaviors to improve their health is excellent.  The question is what role does the employer want to play and is the best vehicle for providing this solution a group health plan?

The financial benefit of providing a wellness program has been hard to measure without a large enough number of employee's to allow experience to be utilized in setting health insurance rates.  Many companies do not fall into this category.  The ironic issue is that companies can have 9 out of 10 of their employee's adhering to a great wellness path and yet they still have negative experience due to significant claims which the premium of the company cannot sustain, simply because some health claims are extremely expensive.

The solution ultimately comes down to use as consumers, employee's, and users of the healthcare system.

I just read the linked article and am concerned about the message we as consultants and advisers will be providing our employer clients.  The concern that I have is centered around the decision that employers will automatically keep their current benefit structure at the expense and assuredly more cost to their employee's.  The costs that are passed along to the employee's usually include lower benefits, too.

Would it make more sense to approach the solution from a consultative position based on cost or coverage or both?

Should we be more concerned about the costs to the individuals or employee's or the employer?

This is a great example of the politics that are influencing the unfolding of healthcare reform.  You now have the 'Conservatives' taking the positions to extend a program that is counter the insurance marketplace.
We can all agree that reform was needed on how healthcare was financed and delivered, however, would any of us argue that our coming healthcare reform was done right?

Now we are adding to the mix the political leaders that are flipping fiscal sides with their position on the PCIP plans.

How can we fix the issues surrounding healthcare without engaging the individual into the position of controlling their future?

In the marketplace today there are differing opinions on the practicality of individual versus group health insurance and the future role that each will have in securing coverage for Americans.

I would propose the the solution to the healthcare begins with each of us as individuals.  We need to allow the market to be transformed by its participants.  iPlanRx believes that through empowerment of the individual a greater good can be achieved both for the individual but also the market.
